Financial Aid Forms
Once you have been awarded financial aid for your college education at CGCC, it’s your responsibility to maintain your eligibility and make sure you’re following up as needed. Sometimes special circumstances arise — such as appealing a financial aid denial, requesting a student loan, or other situation — and you’ll need special forms for those times.
General Document Upload
Use this form to upload any documents requested from the financial aid office that do not have their own specific link.
Consortium Agreement
This form is needed when you are taking classes at CGCC and another Maricopa Community College, Arizona State University or Central Arizona College and you would like to receive your aid based on those credit hours. Before submitting the agreement to the financial aid office you must enroll at the participating school and have completed both a FAFSA and an admissions application at CGC. There are deadline dates each semester to submit a consortium agreement. Please check with the CGCC Important Financial Aid Dates section for these dates.
Satisfactory Academic Progress Appeal
This form is needed when you have been denied financial aid at CGCC due to not meeting the pace of progression or GPA standards of Satisfactory Academic Progress (SAP) but faced extenuating circumstances that prevented you from doing so and wish to appeal.
Academic Plan Review Form
This form is required when your Satisfactory Academic Progress (SAP) appeal has been tentatively approved awaiting an academic plan. You will need to upload this form, along with your academic plan that you receive from your academic advisor to our General Document Upload found above on this page.
Maximum Time Frame Appeal
You are expected to complete your academic objective within an allowable time frame. If you have reached this maximum time frame (e.g., you have already completed a degree or have attempted excessive credit hours), you will need to complete this form for the Financial Aid Office to determine if you are eligible to receive financial aid at CGCC.
Direct PLUS Loan Request Statement
You must complete and submit this form for a Direct PLUS Loan.
Tribal Financial Analysis Request (FNA) Form
Please contact your tribe to begin this process.
Request Additional Loan Funds
Due to circumstances, you may decide your financial aid awards are insufficient, and you would like to request an additional unsubsidized federal student loan. It’s important to remember that the federal government does not pay the interest charges on unsubsidized loans while you’re still in school — you do.
If you are considering taking out an unsubsidized loan, ask yourself the following questions:
- Is borrowing money absolutely necessary?
- Have I cut expenses wherever possible?
- Have I explored other options, such as part-time work?
- What will my loan payments be, and will I be able to afford them?
- Do I need it, or am I borrowing it just because I am eligible?
