Request Forms

You're busy. We get it, and we have a way to help you save time. Fill out one of our online request forms to:

  • Plan classes for your next semester
  • Verify a prerequisite to enroll in a class

Do you need help determining what classes to take next semester?

Continuing students in a declared CGC degree or certificate program can complete our Class & Semester Planning Request Form.

An academic advisor will review your student account and provide class recommendations for your upcoming semester. To view the results of your request, log into your Student Center and check your Advising Notes located under the Academic Progress tile.  

Note: Some requests may be denied if any of the following apply:

  • You are considered a first-time college student 
  • You have not completed CGC’s Enrollment Steps or Coyote Kickoff
  • You are not declared in a CGC degree or certificate program and/or are undecided on your major

 

Have you met the prerequisites for a class but are unable to enroll?

Fill out our Prerequisite Override Form. Staff from the Admissions & Records department will enroll you in the requested course(s), provided you meet the necessary requirements.
