Admissions Forms

Whether you’re getting ready to apply for admission to CGCC or you’re a current student with a special request, the Admissions & Records Office can help. You’ll find the various admissions forms you may need below.


Electronic Forms

  • CGC General Document Upload Admissions & Records
  • Domicile Affidavit (Residency Appeal) – Dependent Student
    • To be completed by dependent students who were determined ineligible for residency.
  • Domicile Affidavit (Residency Appeal) – Independent Student
    • To be completed by independent students who were determined ineligible for residency.
  • Enrollment Verification 
    • To be completed by students to obtain enrollment status information. 3-5 business days. *Up to 15 business days during August and January.
  • Identity Authentication and Declaration of Citizenship or Status
    • To be completed by students who need to submit government-issued photo ID for admissions and proof of lawful presence for tuition purposes.
  • Legal Name/Legal Sex Update Request
    • To be completed by students who wish to update name or legal sex. Additional documentation is necessary.
  • Medical and Compassionate Withdrawal Form
    • This form is intended for requesting a medical or compassionate withdrawal exception. Please ensure that your request includes all relevant documentation that provides additional information to support your medical or compassionate circumstances.
    • Please allow 5 to 10 business days for processing. During peak cycles, additional processing time may occur.
    • All decisions are final.
  • Prerequisite Override Form
    • To be completed by students who need verification to enroll in class(es) that require proof of prerequisites.
    • Please allow 5 to 7 business days for processing.
  • Program Plan/Major Change Request (current students only)
    • To be completed by students who wish to update program plan/major.
  • Refund Appeal Exception Request
    • To be completed by students who withdraw from classes and meet one of the exceptions outlined in the Refund Policy.
  • Student Information Update Request
    • To be completed by students who wish to update their Social Security Number (a signed copy of the Social Security Card is required). Please note, students can change their physical address, phone number and email address in the student services center.
  • Students Under 18 - NEW Concurrent Enrollment Form
    • To be completed by students and parents/guardians prior to enrollment in the first term of classes at CGCC.
    • To be completed by students under the age of 18 wanting to take college credit classes.
    • Parent/guardian consent is required.
    • This form does not apply to students taking classes through the Dual Enrollment, ACE or Hoop of Learning programs.
  • Students Under 18 - Returning Concurrent Enrollment Form
    • To be completed by students under the age of 18 wanting to take college credit classes.
    • Parent/guardian consent is required.
    • This form does not apply to students taking classes through the Dual Enrollment, ACE or Hoop of Learning programs.
  • Transcript Evaluation Request 
    • To be completed by students who would like official college/university transcripts evaluated for transfer credit.
    • Official college/university transcripts must be on file at CGCC prior to submission of this form.
    • Students must be currently admitted at CGCC to receive an evaluation.
    • Please allow a minimum of 6-8 weeks for evaluation results. Requests may take longer during peak times and/or special circumstances.
  • Western Undergraduate Exchange Request (WUE)
    • To be completed by students who are eligible for tuition rates under the WUE program. Eligible students must submit this form along with a copy of their current state driver’s license and/or tax information from their home state.

Processing Your Request

  • It may take three to five business days for our team to review your submitted documents and fulfill your request(s). Please check your Maricopa email for updates.
  • We’ll email your Maricopa email once your document is processed.
  • If we’re unable to process your document, we will notify you of the issue and tell you how to resubmit with the needed information.
